R.I. Chronic Care Sustainability Initiative adds 25 primary care practices

PROVIDENCE – The Rhode Island Chronic Care Sustainability Initiative, which operates patient-centered primary care practices throughout the state, has announced that 25 primary care practices sites have been accepted to join its primary care reform initiative.

As a result, CSI-RI now has 43 primary care practices with 73 total practice sites, serving more than 320,000 Rhode Islanders.

“The number of applications we’ve received from practices to join our initiative tells us that providers are looking for more innovative ways to provide quality care to their patients, and are interested in participating in a program that moves them toward value-based payment,” Debra Hurwitz, co-director of CSI-RI, said in a statement. “We look forward to supporting our new practices as they transform into high-performing patient-centered medical homes, strengthening our efforts to improve the way primary care is delivered and paid for throughout our state.”

CSI-RI promotes the patient-centered medical home, a model of primary care that helps practices improve care quality, access and management, and focuses on team-based, comprehensive and preventative care.

During the CSI-RI 2014 Open Call for Applications, the selection committee reviewed all applications from practices across Rhode Island, following an extensive and objective selection process. Practices were selected based on their track record as agents of change and their commitment to continuous, data driven improvement in heath, healthcare and value.

Data released in May 2014 showed participating CSI-RI practices collectively met every targeted patient heath outcome, including areas of weight management, diabetes, high blood pressure and tobacco cessation, and practices are showing improvement over time in all of the targeted areas.
