United, physician group look to drive patient engagement

PROVIDENCE – Using a new website, mywellview.com, that integrates a patient’s medical records, health expenses and health plan information in one place, UnitedHealthcare of New England has partnered with the Rhode Island Primary Care Physicians Corp. to develop an innovative patient liaison program to help drive patient engagement and improve health care quality and outcomes.
UnitedHealthcare plan participants who see providers at Rhode Island Primary Care are automatically enrolled to participate.
With the click of a button, patients can access their latest lab results, see if they have met their deductibles, check their claims and soon will be able to schedule their medical appointments.
Under the plan, accessible to members enrolled in UnitedHealthcare’s employer-sponsored health plans, patient liaisons at the insurer collaborate with nurse care managers at the physicians’ group, reviewing a patient’s risk factors, preventive tests, services and physician-recommended interventions, to confirm that the patient is following up on those recommendations.
Participating primary care doctors will receive monthly updates on their patients, enabling them to monitor all of the care the patient is receiving.
