University Medicine expands PCMH model

PROVIDENCE – University Medicine has entered into an agreement with Blue Cross & Blue Shield of Rhode Island to roll out its patient-centered medical home model to six more of its practices, BCBSRI announced Monday

Current PCMH practices are located in Providence, but University Medicine is in the process of expanding the methods to additional practices in East Providence, Pawtucket, Providence and Warwick.

“Preliminary data, both locally and nationally, suggests that patients often receive higher quality care when treated in a PCMH,” Dr. Gus Manocchia, senior vice president and chief medical officer for BCBSRI, said in a release announcing the agreement.

“[University Medicine] remains committed to working with us to help improve our members’ overall health, with the ultimate goal of moderating long-term health care costs,” said Manocchia.

According to Dr. Francis Basile, division director of Primary Care at University Medicine, transitioning to the PCMH model allows UM to “deliver better access and to better coordinate care across the spectrum from primary to specialty to hospital care.”

The following practices will transition to the PCMH model:

  • Family Practice

    • Warwick Family Practice, 1035 Post Road, Warwick, 02888

  • General Internal Medicine

    • Barrington Primary Care, 1275 Wampanoag Trail, East Providence, 02915

    • North Main Street Primary Care, 909 North Main St., Providence, 02904

    • General Internal Medicine (GIM), 111 Plain St., Providence, 02903

    • Women’s Health Associates, 111 Plain St., Providence, 02903

  • Geriatrics

    • Geriatrics Practice, 407 East Ave., Pawtucket, 02860
